What is the difference between exosomes and stem cells?

Mesenchymal stem cells are living, multipotent cells that can differentiate and self-renew; exosomes are the 40–150 nm extracellular vesicles those cells secrete, carrying mRNA, miRNA, proteins, and lipids with no nuclear material and no ability to replicate. Both of the materials Akira Biotech distributes come from the same source — Wharton's Jelly in donated umbilical cord tissue — so the decision between them is a decision about mechanism, handling, and characterization rather than about origin.
  • A stem cell is the factory. It engrafts, responds to the local environment, differentiates along its lineage, and keeps secreting signalling factors for as long as it persists.
  • An exosome is one shipment from that factory. Its cargo is fixed at the moment of manufacture, delivered once, and cannot adapt to the tissue it lands in.
  • That trade is the whole comparison. Cells bring responsiveness and persistence; exosomes bring a cell-free, non-replicating, dose-countable product that is simpler to characterize and to ship.

Mechanism: differentiation and paracrine signalling vs cargo delivery

The literature describes three mechanisms for MSCs. Exosomes carry only the third.

Mesenchymal stem cells

  • Direct differentiation into the lineage the product is specified for.
  • Paracrine signalling — the cell secretes growth factors, cytokines, and its own exosomes in situ.
  • Immunomodulation via IL-10 and TGF-β, regulating rather than suppressing the immune response.
  • Low MHC class II expression, which is what makes allogeneic use without HLA matching possible.

Exosomes

  • Cargo delivery only: mRNA, miRNA, proteins, and lipids released into recipient cells.
  • No nucleus, no genome, and no capacity to self-replicate or differentiate.
  • Formed by inward budding of endosomal multivesicular bodies, then released by the parent cell.
  • Cargo profile is set by the parent lineage — which is why the catalog is organized by lineage.

If a protocol calls for something cell-free, there are three preparations to choose between, and they are frequently conflated.

Which one does a research program ask for?

Neither material is presented here as more effective than the other. These are the supply and handling considerations that usually decide it.

Programs that specify cells

  • The protocol depends on engraftment, differentiation, or sustained in-situ signalling.
  • The receiving site can hold cryopreserved material and handle a controlled thaw.
  • Immunophenotype and karyotype are part of the release criteria being recorded.

Programs that specify exosomes

  • A cell-free, non-replicating material is required by the study design or the jurisdiction.
  • Dose needs to be expressed as a countable particle number rather than a cell count.
  • The route is topical, intranasal, or otherwise poorly suited to a cell suspension.
